What Is It?
In 1978 Mattel released this toy based upon the Battlestar Galactica series. The vehicle was equipped to launch a little red missile when a button is pushed.
What Went Wrong?
December 31st 1978 this toy caused a fatality. A 4 year old Boy suffocated when the missile from this toy launched in his throat.
Aftermath?
The item (as well as all of their toys that had missiles) were recalled immediately after the death, and they reworked the toy so that the missiles were glued inside and no longer actually shot out of the craft.
